IDR Recovery Solutions
Clarity at every step.Federal IDR dispute shares · Q4 2025
56%
of all federal IDR disputes initiated for out-of-network emergency and non-emergency services.1
Our team brings extensive experience in negotiation and dispute resolution across the healthcare and legal sectors, applying that background to IDR disputes with a practical understanding of how to pursue additional reimbursement for providers.
A STARTING POINT. NOT THE WHOLE STORY.
You delivered the care. The insurer told you what it's worth. If that payment falls short, your practice shouldn’t have to choose between accepting it and taking on another administrative burden. PosiMed handles the process for you, from eligibility to reimbursement.

Your biller knows your practice. We bring dedicated attention to eligible payment disputes, with clear responsibilities and a personal connection.
We coordinate with your existing billing team to gather claim information and supporting documentation, so everyone knows what is needed next.
A personal contact answers questions and keeps your team updated on the dispute, outstanding information, and next steps.
We follow the dispute through resolution and payment tracking, keeping communications and outstanding actions organized.
The Process
Your biller stays in place. We handle the dispute workflow, coordinate the information we need, and keep your practice informed from the initial review through payment follow-up.
Explore Your Recovery OpportunityWe review records with your biller, check eligibility and deadlines, and identify missing information to assess which claims have a viable federal IDR pathway.
For claims we agree to pursue, we handle open negotiation with the insurer, present the payment position, and track responses and deadlines.
If negotiation does not resolve an eligible claim, we prepare and manage the authorized federal IDR submission, organize supporting evidence, and track the determination.
We track determinations and actual insurer payments, reconcile receipts with your biller, and keep your practice informed about payments received and amounts still outstanding.
THE RIGHT CLAIM. THE RIGHT PATHWAY.
Figuring out which claims qualify shouldn’t become another burden for your team. PosiMed reviews the requirements, identifies missing information, and determines which claims we can pursue, so your practice can focus on patient care.
Not every out-of-network underpayment belongs in federal IDR. The applicable process depends on the item or service, payer and plan, state law, network setting, dates, and the specific claim facts.
Federal IDR overview from CMS ↗A FEW PRACTICAL QUESTIONS
No. PosiMed complements billing, coding, and routine appeals with a focused IDR escalation workflow. Responsibilities are agreed before work begins.
The initial conversation covers your practice, current payment-dispute workflow, and the information needed to assess a potential engagement. A limited pilot may follow if the scope, economics, and process make sense.
Service fees, responsibility for dispute-process fees, reporting, authorization, and follow-up are established in writing before an engagement. Our service fee is based on additional insurer receipts, with separate process costs. See Fees & Cost Structure for details.
No. The business inquiry form is for practice contact details only. Do not submit patient information or claim details. Any later claim exchange requires an agreed, appropriate intake process and applicable agreements.

1. Insurer dispute shares — Q4 2025. CMS supplemental tables, Table 9 (Excel download) reports non-initiating parties or their representatives, aggregated by email domain, for federal IDR disputes initiated for out-of-network emergency and non-emergency items or services. Air ambulance disputes are excluded. CMS publishes rounded individual shares: UnitedHealthcare 24% (171,928 disputes), BCBS Texas 18% (129,662), and Aetna 14% (101,613). The displayed 56% is the sum of CMS’s published rounded shares: 24% + 18% + 14%. Calculating from the underlying counts gives 403,203 ÷ 707,501 × 100 = 56.9897%, which rounds to 57%. This difference results from rounding the individual shares before summing them. BCBS Texas is the Texas entry, not the national Blue Cross Blue Shield system. Dispute volume does not establish payment levels, underpayment, or outcomes. Logos identify the named entities. No affiliation or endorsement is implied.
Back to insurer figures ↑This is our calculation from CMS Q3 2025 public data, not a CMS-published statistic and not a finding about all insurance claims.
The analysis included 1,164,238 line items with QPA greater than $1 and complete, nonnegative initial-payment and cost-sharing fields, excluding bundled component rows. Initial payment plus patient responsibility equaled QPA, rounded to cents, in 93.19% of included line items (displayed as 93%).
Determined federal IDR cases are a selected sample and do not estimate recovery likelihood. Matching QPA does not by itself prove underpayment.

Back to 93% figure ↑3. Radiology prevailing offers — Q4 2025. CMS supplemental tables, Table 14 (Excel download) reports the median prevailing offer for radiology services as 398% of the qualifying payment amount (QPA). This is the Q4 2025 median of prevailing-offer/QPA ratios at the item or service level. Either side can be the prevailing party. These are payment determinations, not amounts actually collected. This population differs from the Q3 2025 initial-payment analysis in box 1. The figures do not measure an increase in payment on the same claims and are not a measured claim uplift. They do not predict any individual claim outcome.
The same CMS Q4 2025 Table 14 reports median prevailing offers of 334% of QPA for emergency medicine, 361% for anesthesiology, and 440% for pathology/lab. These are specialty-specific medians of prevailing-offer/QPA ratios at the item or service level, not amounts collected or outcomes for every claim. They do not imply that other specialties share radiology’s 398% median.
